We do not collect sensitive health information through our ads or lead forms. Detailed dental and medical history is only collected directly by our clinical team once you become a patient, in person or through our secure patient intake process.

• Fill out a contact or booking form on our website
• Call or text our office
• Message us on Facebook, Instagram, or other social platforms
